가. 주요 대외활동 내용
- 유방 재건술 (Breast reconstruction) 성형외과와 유방외과 의료진이 협진 상담을 함으로써 환자에게 가장 적합한 방법의 유방암의 수술적 치료와 더불어 성형외과적 유방재건술이 이루어져 유방절제술만을 시행한 후의 정신적인 상실감을 즉시 유방재건술을 통하여 심미적 만족도를 높여 극복할 수 있도록 하고 있습니다. 크게는 자가조직과 보형물을 이용한 재건술로 나눠 볼 수 있으며, 유방 절제량과 유방 크기에 따른 맞춤형 진료를 실시하여 보다 환자의 만족도를 높일 수 있도록 노력하고 있습니다.
- 종양성형 유방수술 (Oncoplastic breast surgery) 조기 진단 유방암에서 외과와 성형외과가 협심하여 병변 부위의 제거뿐만 아니라 남은 유방 조직에 성형외과적 술식을 응용하여, 종양제거 수술만이 아니라 마치 성형외과적 미용적 유방 수술을 받은 것과 같은 효과가 나타나도록 노력하고 있으며 이를 종양성형 유방수술이라고 합니다. 유방 보존술 단독으로는 병변의 절제 및 유방암의 치료라는 측면에서는 성공적이라 할지라도, 유방의 모양이 미용상으로는 만족스럽지 못한 결과를 초래할 수 있어 병변 부위를 유방성형을 통해 미용 상의 문제를 동시에 해결하려 노력하고 있습니다. 종양성형 유방수술을 통한 유방암의 제거 및 아름다운 유방의 재건뿐만 아니라, 건측 유방의 성형외과적 수술을 통해 양측의 유방의 대칭성을 최대한 보존할 수 있습니다. 따라서 유방암 수술 후에도 양측 유방의 균형 및 이전보다 나은 삶의 질을 가질 수 있도록 연구하여 수술 후의 일상생활 복귀 및 자신감 회복에 큰 도움이 될 수 있도록 준비하고 있습니다.
- 구순 구개열 재건술 (Cleft lip and palate : Cheiloplasty, palatoplasty) 구순구개열 (토순, 언청이, 구순열: 입술이 갈라진 것, 구개열: 입천장이 갈라진 것)은 안면부에 발생하는 기형 중 가장 많은 것이며 전반적인 선천성 기형 중 네 번째로 많은 것으로 지금도 우리나라의 신생아 중 약 510명 중 한 명은 이와 같은 기형을 갖고 태어나고 있습니다. 구순구개열의 치료는 형태와 특성, 얼굴의 성장에 따라 영향을 받을 수 있으며 시기별, 단계별로 나눠 적절하게 시행합니다. 일반적으로 구순구개열 1차 수술은 생후 3개월~18개월 사이에 시행하며 기능적인 부분과 심미적인 측면을 고려해 체계적인 수술 계획을 수립해야 하며 이후 성장기를 거치며 얼굴 변형을 최대한 고려해 2차 수술을 진행합니다. 칠곡경북대학교병원 성형외과에서는 구순 구개열로 진단되면 다른 신체적 기형에 대한 조사와 유전학적 문제에 대한 상담이 이루어집니다. 적절한 시기에 맞는 구순/구개열 수술에서 부터 성장에 따라 필요한 치료(언어치료 등)를 시행하여 아이들의 심리적 충격을 최소화하고 성장이 완전히 끝난 17~18세 경에 안면골 성형술을 통해 더욱 만족스러운 결과를 얻도록 하고 있습니다.
- 레이저를 이용한 흉터 관리 (Scar care using laser therapy) 성형외과적 수술 뿐 아니라 신체표면의 수술적 흉터를 치료관리개념으로 도입되어 수술 후 상처 염증의 관리 및 흉터 관리를 위해 사용되기 시작하였습니다. 색소 레이저, 프락셀 레이저 등 다양한 레이저 기구를 이용하여 흉터의 색감과 질감을 개선시켜 수술 후 환자들이 보다 나은 삶을 지낼 수 있도록 노력하고 있습니다.
- 연부조직 종양 절제술 (Mass excision) 대개 수년 이상 서서히 자라는 종괴(덩어리)가 가장 흔한 증상이며, 대부분 통증 없이 덩이를 호소하나, 드물게는 갑작스럽게 며칠 사이에 눈에 띄게 자라며, 상당수에서 우연히 발견되는 경우도 많습니다. 모든 양성 종양이 수술로 제거가 필요한 것은 아니지만, 종괴의 크기가 커지거나, 종괴가 신경이나 혈관을 압박하거나, 악성으로 변할 가능성이 있는 경우 수술적 절제적 생검술을 통해 정확한 진단과 치료, 추후 발생할 수 있는 문제를 최소화 하도록 노력하고 있습니다.
- 안면 골절 재건술 (Facial bone fracture surgery) 외상으로 인한 안면 골절은 얼굴 형태의 변화 및 기능의 이상도 초래할 수 있습니다. 이에 대하여 칠곡경북대학교 병원 성형외과에서는 초음파 및 컴퓨터단층촬영을 통한 정확한 뼈의 정복 및 고정을 통하여 빠른 안면 골절 회복을 유도하고 있으며, 많은 다양한 환자 수와 경험을 통하여 최선의 치료를 시행하고 있습니다.
